What the form asks for
- The type of investigation, such as surveillance or a background check.
- Your name, company, and email so we can confirm the assignment.
- The subject's name and address, and a photo if you have one.
- Whether to add a Rapid Recon report, which comes back within 24 hours.
- For claims files: the claim number, date of injury, injury, and insured.
